Indiana Office of Medicaid Policy and Planning

Announcements

The Medicaid Nursing Facility Financial (Cost) Report for nursing facilities and their home offices have been revised.  These updated forms can be found in the Long Term Care section of our website.  The instructions have been updated to incorporate the changes made to the forms.

Nursing facility providers are required to complete the revised cost report forms for fiscal years ending on or after April 30, 2011.  For those providers that have already submitted their Medicaid Nursing Facility Financial Report, Schedules E and J (and Home Office Financial Report Schedules R and S) are the only schedules affected by these State form changes.  Please note that only those two schedules (and Home Office Financial Report Schedules R and S, as applicable) will need to be revised using the new cost report schedules.  The due date for resubmission of these Schedules is 6/30/2012.

If you have not submitted your Medicaid Nursing Facility Financial Report, please use the new forms for your submission.  The due date for providers with a December 31, 2011 fiscal year end has been extended from May 31, 2012 to August 31, 2012.  If the Financial Report is not received by August 31, 2012, provider’s rates will be reduced by 10% for untimely filing effective July 1, 2012, per 405 IAC 1-14.6-4(e)(2).  Financial Reports completed on earlier versions will no longer be accepted.

Please note that Schedule X (Schedule of Staffing Data and Statistics) and Schedule Z (Schedule of Special Care Unit Qualifications) have been removed from the Medicaid Nursing Facility Financial Report.  These Schedules are still required to be submitted to report calendar year data and have dues dates separate from the Medicaid Nursing Facility Financial Report.  For reporting calendar year 2011, Schedule X is due August 31, 2012 and Schedule Z is due June 30, 2012.  In subsequent years these Schedules will both be due on March 31st for the preceding calendar year.


Information is now available on our website to assist long term care providers in determining their Medicaid cost report due dates. The Medicaid cost report due date for each long term care program is available under the respective long term care folder.
